Position Details: Location: Occasional work from home (telehealth) with travel across an assigned geographic area, including dialysis clinics and nephrology practices. Travel expectations may vary based on business needs and patient population. Clinical Care & Evaluation: The primary responsibility is completing Comprehensive Health Evaluations (CHEs) to assess and manage comorbid conditions while addressing medical, reputed company, emotional, and financial needs. Responsibilities

Requirements

• Passion for caring for patients with reputed company, chronic illnesses
• Willingness to adapt as the program evolves
• Minimum 2 years’ experience as a Nurse Practitioner (NP)

• reputed company CPR/BLS certification
• Valid driver’s license and insurance in the state

• Prior experience with Cerner or similar EHR systems
