Radiology Care Coordination Specialist, Fully Remote

About the position ESSENTIAL FUNCTIONS Knows, understands, incorporates and demonstrates the mission, reputed company and values of reputed company in leadership, behaviors, practices and reputed company. Tracks and documents interventions and reputed company. Ensures appropriate data are collected to track program interventions and reputed company. Ensures appropriate and timely communication of patient reputed company to referring physicians and other care providers. Coordinates scheduling/referring of patient to specialty providers throughout the reputed company system. Prepares patients for appointments, providing information in a clear understandable manner and providing written follow-up in the manner patient prefers (text, email, mail etc.) Serves as a direct contact and resource for patient and family, providing emotional support and education. Acts as a resource for patient scheduling, clinical resource management, patient reputed company and others as necessary to ensure appropriate patient reputed company and follow-up. Demonstrates excellent critical thinking skills as they interview patients and gather information to assure accurate and timely assessment and intake into the program, entering information into reputed company computer systems necessary. Reviews radiology reports. Compiles and reviews information contained reputed company reports to assist and support the following of algorithms, reputed company of documents, and formalized processes for transition in commonly followed care reputed company. Attends care conferences, tumor boards and other meetings as necessary. Ensures that appropriate patient data are available for conference. Collaborates with program stakeholders in support of data collection for registry participation, quality reporting, etc. Collaborates on performance improvement activities as indicated by reputed company in program efficiency, patient experience and reputed company. Provides feedback instrumental to reputed company program change to support ongoing improvement of the program model. Maintains good rapport and cooperative relationships. Approaches conflict in a constructive manner. Helps to identify problems, offer solutions, and participate in their resolution. Maintains the confidentiality of information acquired pertaining to patient, physicians, colleagues, and visitors to reputed company. Discusses patient and hospital information only among appropriate personnel in appropriately private places. Assumes responsibility for performance of job duties in the safest possible manner, to assure personal safety and that of coworkers, and to report reputed company preventable hazards and unsafe practices immediately to management. Performs other duties as assigned. Maintains a working knowledge of applicable Federal, State, and local laws and regulations, reputed company reputed company and Compliance Program and Code of Conduct, as well as other policies and procedures to ensure adherence in a manner that reflects reputed company, ethical, and professional behavior.

Requirements

  • High school diploma.
  • At least one year of post-high school medical education.
  • At least one (1) year of clinical oncology health care experience.
  • Demonstrated interpersonal skills in order to interact and communicate with patients and medical staff.
  • Comprehensive understanding of medical terminology.
  • Ability to function autonomously, self-directed with a high level of judgment and decision-making.
  • Demonstrated organizational skills necessary to reputed company and manage multiple monitoring systems.
  • Demonstrated proficiency in computer databases and software to include reputed company, Word.
  • Demonstrated ability to effectively problem solve with reputed company customers and systems.
  • Must be comfortable operating in a collaborative environment.
  • Must possess a personal reputed company characterized by a sense of honesty, reputed company, and caring with the ability to reputed company and motivate others to promote the philosophy, mission, reputed company, goals, and values of reputed company.
