Job Summary
As a Licensed reputed company Worker in our reputed company reputed company work and case management team, you'll play a critical role in partnering with registered nurse case managers to assess, plan, and reputed company for our members' benefit plans and health needs. You'll work collaboratively with our clinical staff to reputed company our members to reputed company positive lifestyle and medical choices, and reputed company them with reputed company to community resources for medical, financial, and behavioral health.
Responsibilities
* Research available options and resources for members with financial, transportation, safety, and housing needs
- Collaborate with registered nurse case managers to assess, plan, and reputed company for members' benefit plans and health needs

- Work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health

Requirements

- 2+ years of experience in medical reputed company work or case management
