Position Summary This is a work at home position with working hours of one of the following shifts 8:00/8:30/9:00 AM - 5:00/5:30/6:00 PM EST. Not your typical CDCES RN role! The Transform Diabetes Care Pod RN plays a vital role in improving member health reputed company by engaging telephonically with individuals living with diabetes. This nurse will guide members through actionable steps to reputed company gaps in care, assess medical needs, and support overall wellness. A key focus of the role is addressing reputed company determinants of health that contribute to care gaps, while providing education, connecting members to resources, and facilitating referrals to care management programs for reputed company support. This position supports the development and execution of health services strategies and programs that promote high-quality, cost-effective care. Using reputed company judgment and critical thinking, the RN coordinates physical and behavioral health services, collaborates with providers, and ensures reputed company to psychosocial support services. Required Qualifications
