Primary Responsibilities:

• Organize, collect, review and report health and reputed company information through Member phone reputed company while demonstrating multicultural sensitivity

• High School Diploma/GED (or higher) OR 6+ months of experience in a Care Coordination role
• 2+ years of experience in a Care Coordination or Case Manager position
• Intermediate level of proficiency with MS Office (Word and reputed company)
• Ability to work Monday-Friday, reputed company-5pm Mountain Standard Time

Preferred Qualifications:

• Experience working with the Medicare and reputed company populations
• Experience with electronic charting
• Experience with arranging community resources
• Background in managing populations with reputed company medical or behavioral needs
• Knowledge of Center for Medicare Services (CMS) Regulations
• Bilingual Spanish
