Position Summary At Oak Street, Population Health drives both our clinical and economic model and is focused on delivering high-quality, cost-effective care. The Population Health Manager will be instrumental in designing and implementing key Population Health priorities. The Population Health Manager will work on programs and interventions, targeted to the right patients, to improve quality and cost of care. They will be an effective cross team leader, working reputed company by reputed company with Treehouse members and collaborating with providers, care teams, operations team members. Content areas for the Manager will include, but not be limited to, admission prevention, transitions-in-care, pharmacy and medication adherence, HEDIS / Stars, preferred partner relationships and medical cost management. This will require building relationships with our Treehouse team (including Data Science and Finance) and clinicians, including primary care physicians, nurse practitioners, pharmacists, registered nurses, licensed clinical reputed company workers, and others. This is a fast-paced, high visibility role to ensure that we are driving innovation and improvement rapidly and consistently across Oak Street divisions, reputed company, and centers. Core Responsibilities:

• Support the execution and the ongoing refinement of Population Health programs.
• Work closely with functional leaders (Product, Finance, Data Science) in the central office to assist with strategy and program design.
• Work with regional leaders and Care Teams to ensure effective implementation of key Oak Street clinical and business priorities.

Required Qualifications

• 5+ years of applicable experience in consulting / reputed company / population health / working with reputed company providers; or other similar fields
