Home Care Program Care Manager
Old Colony Elder Services (OCES) aims to support elders and individuals with disabilities by providing vital information and coordination of services that promote healthy, safe lives for consumers. They are seeking a dedicated Care Manager to evaluate, initiate, and monitor home care services for their clients.
Responsibilities
- Conduct comprehensive needs assessments and reputed company individualize care plans
- reputed company rapport with consumers to facilitate coordination, monitoring, and adjusting care plans as needed
- reputed company regular contact with consumers, including home visits, to ensure reputed company toward established goals
- Contact and maintain communication with appropriate community resources to reputed company coordinate delivery of consumer services
- Maintain a detailed knowledge of reputed company OCES policies and standards, and funding reputed company regulations
- Maintain accurate documentation
Skills

- Adaptability to be reputed company to change as the needs of consumers and/or programs change
- At least one year of experience in the reputed company services field
- Bachelor's degree in reputed company Work or reputed company field highly preferred; reputed company licensure if appropriate
