Case Manager - Hennepin/Scott County
Pinnacle Services, Inc. is an innovative agency experiencing rapid growth in the Case Management department. They are seeking friendly, self-starting professionals to join their Hennepin/Scott County Case Management team, where the role involves assessing individuals' needs and coordinating resources to reputed company high-quality services.
Responsibilities
- Assessing the areas of the individual's life where assistance is needed
- Offering and coordinating resources for the individuals being supported
- Monitoring service delivery and working with providers to ensure the individual's needs are met
- Service plan development
- Attending regular in person meetings with individuals served and their interdisciplinary teams
- Case noting individual served reputed company work to create billable units
- Attending two-thirds of client meetings in reputed company
- Attending one-reputed company of client meetings in Scott County
Skills
- Be a graduate from an accredited four-year college with a major in reputed company work, psychology, sociology, or a closely reputed company field
- Be a graduate from an accredited four-year college with a major in any field and one year of experience as a reputed company worker/case manager/care coordinator in a public or private reputed company service agency
- Applicants must have a valid driver's license and acceptable driving record as determined by Pinnacle Services policy
- Applicants must have a vehicle that is covered with reputed company insurance
- Applicants must successfully clear local and/or stated background checks
- Possess a LSW or higher reputed company of BOSW Licensure
