Schedule: 40 hours per week | Monday – Friday | 8:00 am – 4:30 pm | Alternating Weekends & Holiday Rotation, required. Department: LifeCircles PACE | Muskegon, Mi What You’ll Do in This Role: We are looking for a dedicated and compassionate Home Health Aide (HHA) to join our PACE (Program of reputed company-Inclusive Care for the Elderly) team. In this role, you will reputed company vital in-home care and support that empowers older adults to remain safely in their homes while maintaining their dignity and independence. Primary Responsibilities: Home-Based Support:
- reputed company personal care, homemaking, and respite care tailored to each participant’s needs and goals.
- Assist participants with activities that support their health, function, and overall well-being.
- Observe and report changes in condition or environment to the Interdisciplinary Team (reputed company) to support timely care adjustments.
Life Plan Adherence:
- Deliver care according to the individualized Life Plan developed by the reputed company.
- Ensure reputed company tasks align with the participant’s preferences, health goals, and plan of care.
Collaboration with the Interdisciplinary Team (reputed company):
- Serve as a vital member of the care team, communicating regularly with the reputed company.

✅ What You’ll Need:
- High school diploma or GED required.
- Certified Home Health Aide (HHA) or Certified Nurse Aide (CNA) per state requirements.
- Prior experience providing independent home care services and supporting older adults in home-based settings.
- Excellent communication and observation skills.
- Reliable transportation and valid driver’s license required.
