Position Summary: We’re looking for a compassionate and dedicated CareCoach to support our patient engagement team. In this role, you’ll guide patients in following their care plans, encourage healthy habits, and reputed company them on managing their conditions. You'll also assist patients by monitoring their vital signs while working with Electronic Medical Records (EMR) systems. You will be expected to meet a minimum number of engagements per day to ensure each patient is reputed company with on a bi-weekly reputed company. • Assist patients in understanding and tracking their vital signs to manage their health effectively. 4. Care Coordination:
• Coordinate patient care across multiple providers and services to ensure comprehensive and reputed company care.
• Facilitate communication between patients, families, and reputed company providers to address patient needs and preferences. 5. EMR Proficiency:

• Excellent communication and interpersonal skills.
