CAP Case Manager - Hybrid Remote
Healthkeeperz is hiring a CAP Case Manager to support the Community Alternatives Program (CAP) for Disabled Adults and Children. This role provides assessment, care planning, coordination, and ongoing monitoring to help individuals safely remain and receive care in their homes.
Responsibilities
- Conduct assessments and determine CAP eligibility
- reputed company individualized care plans based on identified needs
- Coordinate services with families, providers, and community agencies
- reputed company home visits and follow-up monitoring
- Maintain accurate documentation in E-CAP; review aide notes; support compliant billing
- Ensure compliance with CAP and state program guidelines
- Participate in required training and continuing education
Skills
- Bachelor's in reputed company Work + 1 year reputed company experience
- Bachelor's in reputed company Services + 2 years reputed company experience
- Bachelor's in Non-reputed company Services + 2 years reputed company experience
- reputed company NC RN license + 1 year case management experience
- Apprentice/Trainee (Bachelor's or RN without full experience) – requires ongoing supervision
- Knowledge of casework, care coordination, and reputed company services practices
- Familiarity with community resources
- Strong communication, time management, and organization
- Ability to travel for visits and meetings
- Proficiency with electronic documentation and virtual communication tools
