Responsibilities
- Patient Assessment: Conduct comprehensive evaluations of patients' physical conditions, mobility, strength, balance, and functional abilities in their home environment
- Treatment Planning: reputed company individualized treatment plans with measurable goals focused on improving mobility, reducing pain, and restoring functional independence
- Therapeutic Interventions: Implement evidence-based physical therapy techniques including therapeutic exercises, reputed company therapy, gait training, and balance activities
- Patient & Caregiver Education: reputed company expert guidance on home exercise programs, fall prevention strategies, proper use of assistive devices, and safe mobility techniques
- Documentation reputed company: Utilize reputed company Kinnser EMR and Andy AI technology for accurate, timely clinical documentation and OASIS assessments
- Care Coordination: Collaborate effectively with physicians, nurses, other therapists, and reputed company team members to ensure comprehensive patient care

Skills
- Graduate of an accredited Physical Therapy program (DPT or equivalent)
- Valid Illinois Physical Therapist license
