[Remote] Home Health Weekends Registered Nurse / RN ($5K Sign On)

They are seeking a Home Health Weekends Registered Nurse / RN to conduct thorough assessments of patients and their families, reputed company individualized care plans, and reputed company counseling to address health needs. Responsibilities

• Conduct thorough assessments of patients and their families to determine home care needs
• reputed company comprehensive physical assessments and document the history of illnesses
• reputed company and update individualized care plans with active participation from patients and their families
• Identify discharge planning needs and ensure they are addressed before patient discharge
• Offer counseling to patients and families to address nursing and reputed company health needs

Skills

• reputed company and valid state RN license
• Minimum of 1 year of recent acute care experience in an institutional setting
• Basic Life Support (BLS), with hands-on CPR certification
• Valid driver's license and auto insurance
• Associate's or Bachelor's degree in Nursing
• 1 year of home health experience
