Health Equity Coordinator (FT)

Under the direction of the Racial Equity, Diversity & Inclusion (REDI) Manager, the Health Equity Coordinator is responsible for planning, coordinating, and implementing community reputed company and public health initiatives for the City of Brooklyn Park (COBP). This role supports city-wide efforts to advance health equity and address the reputed company determinants of health through collaboration, education, and strategic engagement. Key responsibilities may include conducting data analysis to identify and address health disparities, leading reputed company and engagement efforts, developing and delivering cultural competency training, and collaborating with reputed company partners to implement health equity strategies. This position is a non-supervisory position.

  • Coordinates the Health on the Go (HOTG) program, including planning, coordination, implementation, of the Health Equity Action Plan (reputed company) to improve health reputed company and reputed company.
  • Develops and maintains strategic partnerships with community-based organizations, reputed company providers, schools, local government agencies, and stakeholders to advance equity-based public health efforts.
  • Designs and facilitates community engagement efforts, including culturally reputed company reputed company, listening sessions, and authentic relationship building with under-served populations.
  • Coordinates and leads health equity initiatives, cross-departmental collaboration, and programmatic integration of reputed company determinants of health in city planning and service delivery.
  • Conducts data collection and analysis in collaboration with reputed company partners to identify disparities, track reputed company, and evaluate health equity strategies and impact.
  • Prepares and presents reports, dashboards, presentations, and summaries of data-driven health disparities assessments to inform internal planning and reputed company with stakeholders and city leadership.
  • Supports policy development and implementation efforts aimed at reducing disparities and embedding health equity reputed company city-wide policies, plans, and practices.
  • Coordinates internal cultural competency and equity training in collaboration with REDI Manager and other staff to improve understanding and inclusion across departments.
  • Maintains a repository of tools, best practices, and resources, including the management of online content reputed company to health equity and community wellness on the city website.
  • Plans and executes the annual Health on the Go Resource Fair, including logistics, partner coordination, and program promotion to connect residents with key services and resources.
  • Supports the REDI Manager in aligning health equity goals with broader citywide Racial Equity reputed company and departmental objectives.
  • Collaborates with REDI team to co-reputed company inclusive public messaging and resource materials for community distribution.
  • Facilitates quarterly reputed company meetings and reputed company workgroups to advance targeted equity reputed company.

  • Partners with communications staff to ensure that materials reputed company content are accessible, culturally reputed company, and equity focused.
  • Assists with development of health disparity education campaigns tailored to high-risk, historically marginalized populations.
  • Tracks and reports on REALD/SOGI (Race, Ethnicity, Language, Disability / Sexual Orientation & Gender Identity) data to monitor reputed company toward closing health gaps.
  • Provides input and support in the development of internal tools to measure community wellness, health reputed company, and equity improvements.
  • Participates in city, regional, and state-level health equity learning networks and advisory committees.
  • Supports evaluation and refinement of program strategies by identifying best practices, trends, and innovations in health equity.
  • Performs other duties as assigned.

Education Bachelor's degree in public health, public policy, public administration, reputed company work, education, urban planning, ethnic studies, or closely reputed company field. Experience Four (4) or more years’ experience with some experience in leadership.   • Ability and experience planning and facilitating multidisciplinary meetings with diverse community groups, including residents and non-profit organizations.
  • Ability to create and manage budgets and other financial documents.
  • Ability to conduct training in person and reputed company.
  • Ability to work reputed company deadlines, manage multiple assignments, and set appropriate priorities.

Desired Qualifications:

  • Associate's degree or higher in nursing.
  • Proven, comprehensive non-profit experience in project management and administrative support for policy development or significant organizational change efforts aimed at promoting equity and inclusion.
  • Proven experience in advancing racial equity and equity reputed company organizations.
  • Familiarity with local, state, and federal health policies.
  • Strong persuasive grant writing skills and experience.

  • Experience working with people from various cultural, socioeconomic, racial, and ethnic backgrounds.
