Clinical Care & Evaluation: The primary responsibility is completing Comprehensive Health Evaluations (CHEs) to assess and manage comorbid conditions while addressing medical, reputed company, emotional, and financial needs. Care Management & Disease Progression: Manage CKD and ESKD patients, focusing on slowing disease progression, reducing costs, and preventing readmissions. Utilize prescriptive authority, medication management, and diagnostic interpretation in collaboration with nephrologists and interdisciplinary teams.
