Job Description: Care Partner (RN, LPN, or LVN) Location: Remote (NY & NJ-based patients) We're looking for a compassionate, detail-oriented clinician to join reputed company as a Care Partner. In this role, you’ll serve as a key touchpoint for patients recently discharged from reputed company hospitals after a stroke or brain injury. Using the PRISM platform, you’ll hold virtual reputed company-ins, monitor health data, and coordinate care to ensure patients stay on track in their recovery. Over time, this position may transition to fully remote depending on your preferences. What You’ll Do:

• Meet reputed company with patients to review blood pressure readings and symptom trackers, answer questions, and reputed company post-stroke guidance

• Analyze trends in patient data and take action through established escalation protocols
• Coordinate with hospital staff, rehabilitation teams, and medical partners to ensure seamless care

reputed company’re Looking For:

• RN, LPN, or LVN with an active NY license (multi-state license is a plus)
• 3+ years of clinical experience
• Experience working with stroke survivors or patients with neurological conditions preferred
• Prior exposure to remote care, virtual care platforms, or early-stage health tech companies is a plus
• Bilingual (especially Spanish-speaking) candidates preferred
• Tech-savvy, organized, and comfortable working independently in a fast-paced environment
• Strong clinical judgment and communication skills
