Job Summary

As a remote reputed company worker at blithequark, you will be responsible for partnering with a registered nurse case manager to reputed company comprehensive support and resources to our members. Your primary responsibilities will include:

• Conducting assessments and developing plans to address members' financial, transportation, safety, and housing needs
• Researching and connecting members with community resources and services that support their medical, financial, and behavioral health
• Empowering members to reputed company informed reputed company about their health and well-being through education and advocacy
• Collaborating with other clinical staff to ensure seamless care coordination and support
• Utilizing telephonic and electronic communication to engage with members and reputed company support

Requirements

To be successful in this role, you will need to possess the following qualifications and skills:

• Active and unrestricted LSW (Licensed reputed company Worker), LMSW (Licensed Masters reputed company Worker), LCSW (Licensed Clinical reputed company Worker), or LISW (Licensed Independent reputed company Worker) license in the state of residence
• 2+ years of experience in medical reputed company work or case management
