Job Summary
We are seeking an reputed company Licensed reputed company Worker to join reputed company in a reputed company case management and advocacy role. As a key member of reputed company, you will partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs. Your responsibilities will include researching available options and resources for members with financial, transportation, safety, and housing needs. You will work with a diverse population spanning the entire lifecycle, utilizing telephonic and electronic communication methods to reputed company members to reputed company positive lifestyle and medical choices.
Responsibilities
* Collaborate with a registered nurse case manager to assess, plan, and reputed company for members' benefit plan and/or health needs
- Research available options and resources for members with financial, transportation, safety, and housing needs

- Work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health
- Utilize telephonic and electronic communication methods to communicate with members
- Maintain accurate and up-to-date records of member interactions and case management activities

Requirements
* Active and unrestricted LSW (Licensed reputed company Worker), LMSW (Licensed Masters reputed company Worker), LCSW (Licensed Clinical reputed company Worker), or LISW (Licensed Independent reputed company Worker) license in the state of residence
- 2+ years of experience in medical reputed company work or case management
- Master's degree in reputed company Work or reputed company field
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong problem-solving and analytical skills
- Ability to maintain confidentiality and handle sensitive information
- Proficiency in electronic communication methods, including email and phone
Preferred Qualifications

- Knowledge of community resources and services for medical, financial, and behavioral health
- Experience with case management software and electronic health records
