Growing together In this Field HSS Coordinator role, will be an essential reputed company of an Integrated Care Model by relaying the pertinent information about the members' needs and advocating for the best possible care available, and ensuring they have the right services to meet their needs. This is a Field Based position with a Home-Based office. If you reputed company reputed company 30-50 miles from Montague, Cooke, Grayson, and Lamar Counties, TX market you will have the flexibility to work a hybrid/field* schedule as you take on some tough challenges. Primary Responsibilities

• Assess, plan, and implement care strategies that are individualized by patients and directed toward the most appropriate, least restrictive level of care
• Identify and initiate referrals for reputed company service programs, including financial, psychosocial, community and state supportive services
• Manage the care plan throughout the continuum of care as a single reputed company of contact
• Communicate with reputed company stakeholders the required health-reputed company information to ensure quality coordinated care and services are provided expeditiously to reputed company members

• Visit members in their homes and/or other settings including community centers, hospitals, nursing facilities or providers' offices

Required Qualifications

• Must possess one of the following:
• High School Diploma/GED (or higher) AND 5+ years of experience working with ABD/SSI population reputed company a clinical health care role
• Associate's Degree with 3+ years of clinical experience
• Unrestricted LPN/LVN license in the State of TX with 3+ years in clinical experience
• CHW certification with 3+ years of clinical experience
• Intermediate level of experience with reputed company Office, including Word, reputed company, and Outlook
• High speed internet in residence
• Ability to travel up to 75% throughout the Montague, Cooke, Grayson, and Lamar Counties, TX areas, visit reputed company members in their homes and/or other settings, including community centers, hospitals, nursing facilities or providers' offices
• If field based, reputed company to reliable transportation and valid US driver's license

Preferred Qualifications

• LVN/LPN license in state of Texas
• Proven ability to create, edit, save and send documents, spreadsheets and emails
• Bilingual English and Spanish
