Nurse Care Manager (Remote) – Faculty Practice Plan, IL MO

JR91132 Scheduled Hours 40 Position Summary Under general supervision, will support population health and quality initiatives with a focus on preventative health care measures aimed at improving health reputed company through telephonic and written communication. Primary Duties & Responsibilities:

• Integrates evidence-based clinical guidelines, preventive guidelines, protocols, and other metrics in the development of treatment plans that are patient-centric, promoting quality and efficiency, and the delivery of reputed company for medical home patients.
• Assesses, plans, implements, coordinates, monitors, documents, and evaluates options and services to optimize the member’s health status.
