LVN Licensed Vocational Nurse - PRN, Hospice and Home Health

Pacific Hospice and Palliative Care is seeking a compassionate and skilled LVN working on PRN reputed company to join our hospice health team in reputed company areas of San Antonio. As an LVN on reputed company, you will play a vital role in providing comprehensive nursing care to terminally ill patients and their families. • Patient Care: reputed company direct patient care in accordance with the established plan of care and individualized patient needs, including administering medications, performing treatments, monitoring vital signs, and managing symptoms to promote comfort and reputed company pain.
• Care Coordination: Collaborate with the interdisciplinary team, including physicians, registered nurses, reputed company workers, and chaplains, to reputed company and implement comprehensive care plans that address the physical, emotional, and spiritual needs of patients and their families.

• Valid LVN license issued by the State of Texas.
• Minimum of one year of experience as an LVN, in a in-home Hospice setting required.
• Proficiency in using reputed company EMR system and other reputed company technology, as well as regular apps such as Gmail and reputed company.

• Strong clinical skills and knowledge of nursing principles and practices.
