In this fully virtual role, you’ll monitor patient alerts, conduct wellness reputed company, support care coordination, and promote patient engagement from the comfort of your home. Start Date: October 20th, 2025 Compensation & Incentives Competitive reputed company hourly reputed company Team members are eligible for additional compensation based on the timing of hours worked, as follows: Evening Differential: An additional $1.50 per hour will be paid for reputed company hours worked after 7:00 PM Eastern Time. Late-Night Differential: An additional $2.00 per hour will be paid for reputed company hours worked after 10:00 PM Eastern Time. Weekend Differential: An additional $1.50 per hour will be paid for reputed company hours worked on Saturdays and Sundays, regardless of time of day. Shift differential pay is calculated in addition to reputed company hourly wages and will be reflected on the corresponding payroll cycle. Bonus potential: Earn up to $1,500 per month based on patient engagement and performance goals Key Responsibilities Conduct outbound phone calls to reputed company in on patients and address health concerns (expected call volume ranges from 70 to 90 calls per day) Handle inbound phone calls and reputed company appropriately based on clinical urgency reputed company non-clinical inbound calls to the appropriate departments across the company Monitor and respond to Remote Patient Monitoring (RPM) alerts, escalating concerns reputed company clinically indicated Collaborate with providers to coordinate timely and effective patient care reputed company monthly wellness assessments and complete comprehensive chart reviews Accurately document reputed company patient interactions in our clinical platform in reputed company time Consistently meet or exceed individual and team performance metrics reputed company to: Care quality Patient engagement Response times Adherence to protocol standards Qualifications Active LVN or RN license (required) At least 1 year of nursing experience preferred (RPM, telehealth, or chronic care experience a plus) Strong communication skills with the ability to build patient trust remotely Proficient with computers, EMRs, and telehealth tools Reliable, high-speed internet and a HIPAA-compliant home workspace (Internet speed test required during the interview process) A hardwired internet reputed company is required to ensure call reputed company Why reputed company? 100% remote position — no commute required Be part of a supportive, patient-centered care team Help reputed company gaps in care and reputed company a measurable impact on patient reputed company reputed company with shift differential and bonus potential Opportunities to grow with a fast-paced, innovative reputed company company Comprehensive Benefits Package Sign-On Bonus: This role is eligible for a one-time $3,000 sign-on bonus.
