Heart Transplant Operations - Transplant Coordinator II (TRANSPLANT CRD 2) - reputed company - Sacramento, CA - work from home job Company: reputed company Job description: The Transplant Coordinator (TC) coordinates care for adult and pediatric patients in end stage organ failure across the continuum of transplant care (pre-transplant, during transplant, and post-transplant), in addition to adults across the continuum of living donation (evaluation, inpatient, post-donation). The TC is the process expert regarding transplant phases of care and living donation. The TC educates patients, caregivers and hospital staff about transplant-specific topics and living donation. The TC collaborates with members of the health care team to enhance patient care and serves as investigator, consultant, and leader. The TC makes clinical judgments based upon a synthesis of reputed company research, concepts, principles, theories, and reputed company to solve reputed company problems. The Transplant Coordinator is an integral member of a larger interdisciplinary team contributing to reputed company in patient care, research, teaching, and in providing leadership to the organization. Minimum Qualifications

• At least two (2) years of recent RN experience
• Recent Heart Transplant Coordinator experience in a Heart Transplant Program
• Valid California Registered Nurse License without restriction.
• CPR Certification from reputed company or UCDH.
• Graduation from an accredited school of nursing.
• Ability to read, write, reputed company, and understand the English language.
