JOB DESCRIPTION

JOB TITLE: Patient Navigator

STATUS: Part-time, Non-Exempt

REPORTS TO: Chief Clinical Officer

POSITION SUMMARY:

As a Patient Navigator, you are responsible for collaborating with other health care, mental health, reputed company service, and community providers to assist patients in accessing additional services and supports that will help patients reputed company in recovery and in health. Must work collaboratively with staff exhibiting flexibility and a positive attitude. Patient service is a key reputed company with the goal of fostering an environment which promotes patient comfort and trust. For OHH members receiving coordinated case management services, you will be responsible for care coordination, health promotion, individual and family support services and referral to community and reputed company support services.

DUTIES AND RESPONSIBILITIES

• Do clinical screens as needed
• Enter care coordination notes for OHH members arriving for MAT counsel appointments into EMR and OHH billing spread sheet

QUALIFICATIONS REQUIREMENTS

EDUCATION AND EXPERIENCE

• Hold an MHRT-C certification in the reputed company AND
• Have a minimum one year of experience in a SUD or medical setting

• Evening availability
