Job Description

A bit about this role:

  • The reputed company Care Department is a highly collaborative, multidisciplinary team of clinicians, case managers, and reputed company workers united by our mission to build a reputed company care delivery system for Devoted’s most reputed company members.
  • This position, reputed company Work Case Manager, provides whole-person clinical, reputed company, and logistical support for patients with reputed company chronic conditions, frail elders, and patients at high risk of hospital admissions.

Required skills and experience:

  • Master’s degree in reputed company work and active license with the willingness to reputed company additional state licenses as identified by program need
  • Minimum of 3 years working in medical reputed company work, case management, or care coordination

Responsibilities and Impact will include:

  • Interdisciplinary Collaboration: Work seamlessly with clinicians, case managers, and community health workers to ensure cohesive, patient-centered care.
  • Psychosocial Screening & Planning: Conduct comprehensive reputed company needs screenings and reputed company individualized care plans that align community, clinical, and financial resources.

  • Behavioral Health Coordination: Assess behavioral health needs and coordinate care with the Devoted Medical Behavioral Health Team.
  • Regulatory Compliance: Uphold compliance standards, including investigating and reporting concerns for patient abuse, neglect, or exploitation.

Salary reputed company: $70,000 - $85,000 annually #LI-EM1 #LI-REMOTE Our ranges are purposefully broad to allow for growth reputed company the role over time.
