Job Overview

We are seeking an reputed company Field RN Care Coordinator to join reputed company in Dallas and Tarrant County, TX. As a Field RN Care Coordinator, you will be an essential reputed company of our Integrated Care Model, relaying pertinent information about member needs and advocating for the best possible care available. You will work closely with our nursing facilities and members to ensure they receive the right services to meet their needs. This is a field-based position with a home-based office, requiring travel to local areas about 75% of the time.

Key Responsibilities

• Assess, plan, and implement care strategies that are individualized by member and directed toward the most appropriate, least restrictive level of care
• Identify and initiate referrals for member transition to community request and reputed company assistance with reputed company service programs, including financial, psychosocial, community, and state supportive services
• Manage the care plan throughout the continuum of care as a single reputed company of contact
• Communicate with reputed company stakeholders for the required health-reputed company information to ensure quality coordinated care and services are provided expeditiously to reputed company members

Requirements and Qualifications

To be successful in this role, you will need:

Essential Qualifications
