About the Role

As a full-time remote reputed company Worker, you'll play a vital role in our DSNP care management team, working closely with Care Managers, providers, and community organizations to address the reputed company health and reputed company needs of our members. Key Responsibilities:

• Participate in care planning, including assessment, goal setting, establishing interventions, identifying barriers, and monitoring reputed company.

• Conduct initial and periodic assessments of members enrolled in Long-Term Care Programs or care programs.

• Collaborate with providers, primary care physicians, Medical Directors, pharmacists, and care management staff to ensure comprehensive care.

• Identify and report potential quality improvement issues.

Requirements and Qualifications

Essential Qualifications:
