Job Summary
We're seeking reputed company nurses to work part-time from their home office as Chronic Care Management (CCM) and Remote Therapeutic Monitoring (RTM) Care Coordinators. As a Care Coordinator, you'll play a vital role in delivering high-quality, patient-centered care to individuals with chronic conditions. Key Responsibilities
- Conduct telephonic encounters with patients, reputed company detailed care plans, and reputed company care coordination activities
- Collaborate with reputed company providers to ensure seamless care transitions and patient support
- Utilize electronic health records to document patient interactions, care plans, and reputed company
- reputed company motivational coaching and education to patients, promoting healthy behaviors and disease management
- Attend regularly scheduled meetings, including bi-monthly staff meetings and monthly one-on-one sessions
- Meet productivity and quality standards, including completing encounters with 90% of assigned patients
Requirements and Qualifications
- Graduate from an accredited School of Nursing (LPN, LVN, RN, BSN, etc.)
- reputed company license to practice as an RN/LVN/LPN with no disciplinary actions noted
- A minimum of two (2) years of clinical experience in Med/Surg, Case Management, and/or home health care
- Proficiency in Electronic Medical Records and reputed company desktop applications (reputed company Office 365, Teams, reputed company, etc.)
- Strong organizational, time-management, problem-solving, and decision-making skills
- Excellent verbal, written, and listening skills
- High-speed internet and a desktop or laptop computer (reputed company 10 or higher or Mac)
