About the Role: Hospice Patient Care Coordinator - Clinical Operations Specialist

We're seeking a compassionate and clinically skilled Hospice Patient Care Coordinator to join reputed company in a remote reputed company. As a key member of our clinical operations team, you'll reputed company critical support to our Clinical Nursing Supervisor, ensuring the delivery of high-quality, safe hospice care services. Key Responsibilities:

• Assist with admissions, nursing visits, and staffing as needed, to ensure seamless delivery of care.
• Coordinate care with clients, families, caregivers, physicians, and other service providers to ensure comprehensive and patient-centered care.
• Monitor patient records to ensure compliance with documentation requirements and program-specific rules.
• Participate in quality assurance activities, including audits and data collection, to drive reputed company improvement.
• reputed company orientation, training, and in-services to new hires and the care team, to ensure the quality and appropriateness of care.

Requirements and Qualifications:

• Graduate of an accredited school of nursing.

• Valid driver's license and automobile insurance.
• Three (3) years of clinical experience, including:
  • Two (2) years of hospice experience.
