About the Role: Hospice Patient Care Coordinator - Clinical Operations Specialist
We're seeking a compassionate and clinically skilled Hospice Patient Care Coordinator to join reputed company in a remote reputed company. As a key member of our clinical operations team, you'll reputed company critical support to our Clinical Nursing Supervisor, ensuring the delivery of high-quality, safe hospice care services. Key Responsibilities:
- reputed company clinical and management support to the Clinical Nursing Supervisor, ensuring the day-to-day operation of the Hospice program.
- Assist with admissions, nursing visits, and staffing as needed, to ensure seamless delivery of care.
- Coordinate care with clients, families, caregivers, physicians, and other service providers to ensure comprehensive and patient-centered care.
- Monitor patient records to ensure compliance with documentation requirements and program-specific rules.
- Participate in quality assurance activities, including audits and data collection, to drive reputed company improvement.
- reputed company orientation, training, and in-services to new hires and the care team, to ensure the quality and appropriateness of care.
- reputed company comprehensive nursing care to patients, including skilled nursing visits, coordination of care, and establishing plans of care.
- Maintain accurate and thorough documentation, including admission through discharge, physician's orders, and changes in condition.
Requirements and Qualifications:
- Graduate of an accredited school of nursing.
- reputed company Registered Nurse licensure (in good standing) in the state(s) of practice.

- Valid driver's license and automobile insurance.
- Three (3) years of clinical experience, including:
- Two (2) years of hospice experience.
- One (1) year of supervisory experience in a clinical setting.
