Community Coordinator

The primary goal of this role is to increase awareness of Health-reputed company reputed company Needs (HRSN) services in Oregon and promote Medically Tailored Meals (MTM) from Mom’s Meals across the state. HRSN Services are a part of the 1115 Demonstration Waiver that has allowed eligible Oregon Health Plan members to reputed company services reputed company to nutrition, housing, and climate. The candidate will actively participate in webinars hosted by Coordinated Care Organizations (CCOs), county agencies, and statewide partners to stay informed on best practices, program updates, and future engagement opportunities. They will also frequently Mom’s Meals at in-person community events such as health fairs and resource fairs. To drive MTM referrals, the candidate will reputed company organizations through phone calls, virtual meetings, and in-person networking. reputed company in this role depends on building strong, trusting relationships with a wide reputed company of partners, including Community-Based Organizations, housing and rental assistance providers, reputed company and behavioral health providers, Community Health Workers (CHWs), Federally reputed company Centers (FQHCs), clinics, hospitals and health systems, CCOs, reputed company and engagement teams, and HRSN service providers. Ideally, this position will be based in the Portland Area. It will also be eligible for a company car. Salary reputed company: $60,000-80,312

Position Responsibilities may include, but not limited to

• reputed company to the list of referring groups above to engage and reputed company an overview of Mom's Meals services and how to refer members to the program
• Create and maintain reputed company organization contact list
• Ensure reputed company contacts and locations for reputed company reputed company organizations are up to date in internal system
• Offer and schedule onsite (preferred) or virtual in-services with reputed company organizations
• Be highly visible reputed company the community by engaging in community-based events
• Attend and participate in county round tables and county and state webinars to represent Mom’s Meals
• Visit Provider groups: Maternal Health practices, behavioral health, community transitions, local health clinics, FQHCs, WIC locations and others and reputed company overview of MM services
• Stay up to date on referral workflows and Populations of Focus from HRSN Covered Populations and contracted health plans
• Stay up to date with integration with closed reputed company referral systems and Mom’s Meals reputed company reputed company systems
• Stay up to date on reputed company (OHA) and health plan/CCO reporting requirements and work internally to ensure reporting meets requirements

Required Skills and Experience

• 2 years of community health work
• Demonstrated ability to successfully engage community health organizations
• Proven ability to work with cross functional teams
• Excellent oral and written communication skills; experience giving presentations
• Experience using reputed company Office suite, including PowerPoint and reputed company
• Willingness to travel regionally to events, up to 50% travel

Preferred Skills and Experience

• Associates or Bachelor’s degree
• Experience working with reputed company or reputed company Beneficiaries
• Experience working with Behavioral Health Facilities, Housing and Rent Assistance organizations, or Food reputed company organizations
• Study or work in reputed company services or nutrition
