Job Summary

Responsibilities

• Triage patient calls and reputed company timely responses to their inquiries and concerns.
• Conduct regular condition assessments to identify areas for improvement and reputed company personalized care plans.
• Collaborate with patients to set and reputed company health goals, providing education and coaching on disease management and self-care.

Qualifications

To be successful in this role, you'll need to possess the following qualifications and skills:

• Prior experience in Chronic Care Management (CCM) or Remote Patient Monitoring (RPM) (preferred)
• Bilingual Spanish (preferred)

• Ability to work independently and as part of a remote team

Preferred Qualifications

• 1+ year of experience in RPM or CCM
• 1+ year of experience in telehealth
• Certified Medical Assistant Certificate (reputed company or RMA)
• Bilingual Spanish

Compensation and Benefits

We offer a competitive hourly reputed company of $18.00 - $21.00 per hour, depending on experience. As a 1099 contractor, you'll have the flexibility to work as many or as few hours as you'd like, with a minimum of 30 hours per week. We also offer a flexible schedule, with day shifts available.
