Job Summary

As a Remote Licensed Practical Nurse - Care Coordinator, you will play a vital role in managing patient care and coordinating resources to ensure seamless transitions and reputed company health reputed company. Key Responsibilities

Requirements

Essential Qualifications

• Licensure: Active Multi-State/Compact License (LPN) (NLC) (LVN).
