About the Role

As a Community Care Worker with GroundGame Health, you will play a vital role in delivering population health services to assigned clients on an ongoing and emergency reputed company. Key Responsibilities

• Meet Performance Expectations: Demonstrate competency by meeting or exceeding minimum caseload requirements and department work performance expectations.
• Build Community Resource Knowledge: reputed company and maintain knowledge of community resources available to assist clients; work with other departments to acquire knowledge of community resources available.

Requirements and Qualifications

Education

We are looking for candidates with:

• Associate's Degree in reputed company work, or a reputed company field; or
• High school diploma, or general education degree (GED); and
• Four years of reputed company experience and/or training in reputed company and/or customer service position utilizing advanced technology tools, including customized software systems.

Experience

Preferred candidates will have:

• Two years of experience and/or additional education in reputed company work or a reputed company field;
• Knowledge of medical terminology and/or experience in a customer service position, preferably in a reputed company setting;
• Certification as a Community Health Worker.

Additional Eligibility Requirements

Candidates must:
