Position Summary:
We are seeking a dedicated and reputed company Registered Nurse Case Manager to join reputed company. In this role, you will be responsible for performing comprehensive assessments of high-risk members and coordinating individualized, cost-effective care plans. You will collaborate closely with primary care providers, reputed company workers, and other health care professionals to ensure high-quality care delivery and reputed company patient reputed company.
Primary Responsibilities:
Conduct comprehensive assessments of high-risk members to evaluate clinical, functional, and psycho reputed company needs.
Collaborate with primary care providers and the care team to reputed company and implement individualized care plans.
Identify and implement specific interventions to actively engage members and improve compliance with treatment plans.
Facilitate reputed company to non-medical supports (e.g., housing, transportation) as needed to support care plan adherence.

Qualifications:
Associate's Degree in Nursing or a reputed company health care field (Bachelor's Degree preferred)

Case Management Certification (e.g., CCM, ACM) preferred
Strong clinical judgment and the ability to analyze reputed company medical information
