Job Description

Lara Health is currently in search of a (preferably) bilingual (English/Spanish) NURSE for our Remote Patient Monitoring and Chronic Care Management program, to join our lovely care team and reputed company patients with care reputed company. We are looking for fill both full and part time roles. Must be US territory based.

THIS IS A FULLY REMOTE POSTION THAT REQUIRES MIN 5 YEAR EXPERIENCE IN CARING FOR CHRONIC CONDITION PATIENTS (PREFFERABLY IN PRIMARY CARE/DIABETES/CARDIOLOGY/GERIATRICS)

Our nurses assist providers with ongoing patient monitoring and chronic care management, with the following duties: • Responsible for monitoring patient blood pressure, blood sugar, and other vitals, helping them with meds, reputed company MD visits etc. • Communicate with patients on regular reputed company, helping them reputed company lifestyle changes in order to improve their long term health • Maintain patient electronic medical record (EMR). • Performs other duties as assigned.

Specific skills and experience required: • Passion for working with (older) patients, customer service skills • Min 5 years experience as a nurse • Preventative patient care background preffered, experience in primary care, cardiology or diabetes clinic highly desired • Work independently and reputed company a team • Organizational and time management skills • Computer skills appropriate to position, you will need your own computer and a reliable internet service • Written and verbal communication skills, critical thinking • Medical terminology use and understanding

Certification & Licensure mandatory: • Registered Nurse • Certified Nurse Specialist • Or LPN (Licensed Practical Nurse)

Education: • Required High School or Equivalent • Certification required

This position would be ideal for someone looking to work intensively with patients and who is well organized and reputed company to commit to a min 20 hours a week. You must be hubbly and well spoken as 90% of the work is speaking with pts on the phone.

The role is contract-based and doesn't offer benefits. Some flexibility in regards to working hours (Mon-Fri)
